Dialog to receive names and checked options

I would like to build a script that first opens with a dialog where the user(me) can input the client’s name and check 4 different options.

Engagement Shot
Negatives
Paper proofs
DVD

According what the user checked different parts of the scrip will be triggered.
Is this possible? How would I do it?

Why not employ a list dialog and “allow multiple selections”. I hope this helps.
